    Regarding:

    My first question, I assume Live-view bandwidth for 1 camera for 4mp is 4Mbps, 200 cameras would require 200x4=800Mbps. Is this 800Mbps will be handled by Nx Server A alone, or Server A will only handle the bandwidth of the cameras connected to it.

    First, in general, 4 MP camera, will consume less bandwidth. But the load only applies to cameras on that specific server, so in your example 50 cameras per server. The most important limitation that has to be considered is the maximum of 2000 simultaneous TCP connections per server. See also THIS support article.

    Regarding:

    My second question, from Nx Client specification, the maximum grid for one monitor is 64. However, since user can open up multiple windows, can the user perform 64 split screens x 4 monitors = 256? Can the Nx Client allow this type of configuration? Please ignore the workstation hardware limitation as the end-user insist on this and the performance of the workstation is under his responsibility.

    Yes, he can open multiple windows. In THIS support article we provide some recommendations regarding hardware specifications for the Desktop client application. We even could increase the max number of items in the layout if he wants, completely useless, but if the customer wants it, we can do change the value for him is we get remote access to his clients.  

    For example; below 256 cameras on a single layout:

    If this set up is intended to act as a video wall, specific hardware is recommended, for example the Bluebox.video hardware which was designed and made for this purpose.
